I hereby agree and consent as follows. 

 

I consent and authorize IRENE’S ENTROPY to use my likeness in any photograph, video or other digital media (“Photos”) taken or to be taken on in any and all of its publications, including print or web-based content. 

 

I consent and authorize the use of my contributed content (i.e. testimony of personal information) to be used as creatively directed and determined by IRENE’S ENTROPY.

 

I irrevocably grant IRENE’S ENTROPY all rights and interest in the copyright of my contributed content. 

 

I waive any rights for approval or inspection of the final product.  

 

I understand and agree that all content submitted to IRENE’S ENTROPY for inclusion in a broader creative work is the property of IRENE’S ENTROPY, and will not be returned to me.

 

I understand and agree that contribution to IRENE’S ENTROPY of personal information may be used as part of an anthology, for promotional and/or marketing efforts, and in any other creative endeavor as determined solely by IRENE’S ENTROPY.

 

I acknowledge that I am not entitled to any compensation and/or royalties with respect to the use of the my contributed content. 

 

I understand that nothing in this Release shall be interpreted to create a joint venture nor a joint partnership. I am simply a willing participant in a creative project for which IRENE’S ENTROPY shall have final creative control, copyright and ownership.  

 

I understand that IRENE’S ENTROPY shall be the sole owner of any derivative work which contains my contributed content in whole and/or in part. 

 

I agree to release and forever discharge IRENE’S ENTROPY and affiliates, successors and assigns, officers, employees, representatives, partners, agents and anyone claiming through them, in their individual and/or corporate capacities from any and all claims, liabilities, obligations, promises, agreements, disputes, demands, damages, causes of action of any nature or kind, known or unknown, which I, and anyone claiming on behalf of me, may have or claim to have against Releasee in connection with this Release.

 

This Agreement shall be governed by and construed in accordance with the internal laws of Illinois without giving effect to any choice or conflict of law provision or rule.

 

I have carefully read and fully understand all the provisions of this Release Form and am freely, knowingly and voluntarily signing by checking the terms and conditions box and submitting this form.
